When mailing a check, please make sure to include any specific information on what you would like your donation to support (i.e. education programs, music enrichment, community events), or if you would like to designate your gift as a tribute in honor or in memory of someone.
GIFTS FROM DONOR ADVISED FUNDS (DAFs)
Contact your Donor Advised Fund administrator to make a gift to FHSR.
You may need our mailing address (PO Box, Chicago, Il 60618) and federal tax ID (36-6082810).
Your DAF administrator should include your name and mailing address with your donation so that we can properly acknowledge your generosity.
